Form 1095-C

Employees eligible for the health system medical plans can download Form 1095-C, a tax form that reports your healthcare coverage month by month for the previous calendar year.

1. Log in to the bswift website.
2. Scroll to the Suggested for You section.

Click Employee File on Review your 1095-C form tile.
